Workers Compensation

In the event that you experience a work related injury or illness, contact your Supervisor and/or Human Resources immediately. Human Resources can be reached at 909 793-3115, ext. 105 or 108.

1. Injured employee/worker notifies supervisor.

    

2. Supervisor or injured employee worker immediately calls Company Nurse at 1-888-375-0280. CRY-ROP’s group code is CRYRO.
3. Company Nurse may refer you to HealthPointe, located at:
   290 N. 10th St. #100
   Colton, CA 92324
4. Contact Human Resources at (909) 793-3115 ext. 108 and advise them that an injury or illness has occurred.
5. Submit all forms to Human Resources as soon as possible.
6. When follow up medical appointments are necessary, please make sure to keep the appointments.
7. Follow up with all correspondence you may receive from Sedgwick Services Group, Inc.
8. Follow up with Human Resources if you have any questions or concerns.
